Step 1:
Complete the registration Google form online.

Step 2:
Venmo $55 non-refundable fee to @MJECheer (or email info@mjecheer.com if you prefer to drop cash or check). Please list your cheerleader's name in the payment comments!

Step 3:
Complete all FIVE forms below. Send picture of completed forms all in one email to info@mjecheer.com with your cheerleader's name in the subject line. Please click on each item below to access and complete. Visit each form link below to print out and complete.

Form 1:  GHSA pre-participation physical completed and signed (4 pages long)
Form 2 : Transportation & photo release
Form 3: Consent to treat medical release
Form 4: Concussion awareness form
Form 5: Cheerleader & parent agreement

Step 4:
Once all necessary steps are completed, you will receive a confirmation email clearing you for tryouts! The email will also contain more details.
